What’s Included in the Consent Form
Client Information
Clients will be asked to answer questions about their medication use, allergies, surgical history, and prior experience with extensions and eyelash growth products. This information helps evaluate the procedure’s safety and identify any factors that could cause the extensions to fall off prematurely.
Procedure Details
One of the most important elements of obtaining consent is disclosing what the procedure entails. To cover this responsibility, the consent form ensures clients understand the following:
- Application Process – How the lashes will be applied, how long the procedure takes, and what the client is expected to do (e.g., remove contact lenses, lie still with eyes closed).
- Potential Risks – Side effects such as allergic reactions or eye infections caused by the tools, chemicals, and other products used during the procedure.
- Care and Maintenance – Guidelines on maintaining the extensions, including activities the client should avoid.
Waiver of Liability
The release form includes an agreement where the client holds the salon and technician harmless for any side effects or other issues caused by the extensions, except if the procedure was performed incorrectly.
