The form is often used for companies that charge customers for memberships or subscriptions on a weekly, monthly, quarterly, or yearly basis. Once the form is signed, the merchant can continue charging the customer’s card indefinitely until either the customer or merchant cancels it.

Getting the customer’s approval with the form helps in building customer trust, and goes a long way in helping merchants win chargeback disputes, should they occur. Using a physical authorization form is most often used by brick and mortar establishments, as digital (software) authorizations typically only require the customer to click “I understand” at checkout. To avoid the potential of fraud or stolen customer information, it is an obligation of the merchant to ensure all authorization forms are stored securely and can only be accessed by certain employees.
